Once a year we ask you to make an appointment with your GP to complete your Diabetes Annual Cycle of Care.

HOW YOU CAN HELP REDUCE YOUR OWN RISK…

  • By exercising 40mins per day 5-6 days per week if physically possible
  • By sticking to a Low GI, Low Saturated Fat diet and watching your portion size
  • By not smoking
  • By not drinking more than 2 alcoholic drinks most days and having at least 2 alcohol free days per week
  • By taking your medication as directed
  • By having regular health checks with your GP specifically for your diabetes
  • By having diabetic blood tests twice a year
  • By monitoring your own diabetes as recommended by your doctor
  • By completing your Diabetes Annual Cycle of Care with the Practice Nurse EVERY year

WHY IS IT IMPORTANT TO LOWER THE RISK OF HEART DISEASE & STROKE?

According to Diabetes NSW:

  • 75% of diabetics die from heart disease, including heart attack and stroke
  • Diabetics are 6 times more likely to develop atherosclerosis than non-diabetics.

HOW CAN AYR MEDICAL GROUP HELP YOU REDUCE THIS RISK?

  • HbA1c Goal = less than 7.0
  • Cholesterol Goal = total less than 4.0

YOUR HEALTH CHECKS
We can help you manage your diabetes most effectively when we regularly review your health. If your diabetes is well controlled , you need a review at least twice a year and more often if your diabetes is not so well controlled.

WHAT WE CHECK AT THE VISITS

EVERY 6 MONTHS:

  • Blood Pressure
  • Weight/Height/Body Mass Index
  • Feet
  • Cholesterol (triglycerides & HDL)
  • HbA1c

EVERY 12 MONTHS:

  • Kidney check (microalbumin)
  • Review of medication, diet, exercise and smoking status
  • Eye examination

You will receive a reminder in the mail when your next diabetes review is due. If there is a Pathology request form with the reminder please have the blood tests BEFORE you come for your appointment.
